To complete enrollment please fill out the forms provided:

• Parent Consent Form

• Medical Form

• In addition to the above medical form, you must also submit a physical from your child’s doctor office.

• If your child has been vaccinated, you must provide proof of vaccination. A photo of the vaccine card can be texted to 917 617 8775, emailed, or brought in on the first day of Hudson Scholars. If you have provided proof of vaccination to Mr. Sykes in the last two months, you do not need to send him anything again.
